We will discuss special instructions about your appointment with you during the first phone conversation.
What to Expect
As you enter the offices of Mountain Region Family Medicine, you will be greeted with a friendly smile. Our receptionist will ask to see your insurance member card and identification. We will scan your information into your electronic medical record chart. We may also ask to take your photograph; this will be added to your electronic chart.
Please come prepared to pay any co-pays, deductibles, or co-insurances at the time of your visit.
If you have not previously completed our new patient questionnaire you will be asked to fill out a Patient Registration Form and a Medical Information Form. These forms are available to print and complete from our website if you would like to fill them out in advance. In addition, we may ask you to complete a Medical Records Release Form, which allows our office to obtain medical records from your previous physicians. Once you have completed the questionnaire, you may give this to the receptionist who will notify the nurse that you are ready to move to an exam room.
If you are a new patient, please be sure to bring your medications or a list of the medications you are currently taking along with your insurance card(s). Established patients should bring new insurance cards when applicable. We may ask you to complete a new Patient Registration Form during your first visit of the year.
Next, a nurse will greet you and escort you to one of our exam rooms, where your weight, blood pressure, temperature, heart, and respiratory rate will be recorded into your electronic chart. You will then be asked about your health concerns for the visit.
Depending upon the nature of your visit, the nurse may provide you with a gown, or ask you to disrobe your child. With a gentle knock at the door, your provider will be in to see you.
After Your Visit or Procedure
The physician may request a follow-up visit approximately two weeks after a blood draw to go over the results. Following a test or procedure done outside of the office, you will receive either a phone call or a letter regarding the results within two weeks. Please contact our office if you have not heard from us within two weeks of your test or procedure.
During your visit, our nursing staff will schedule any tests or procedures that will be done outside of the office. If your insurance company requires us to obtain a prior authorization before a certain test or procedure, please allow us 24 to 48 hours to schedule the appointment. We will contact you with the appointment information once it has been scheduled.
